REGISTRATION INSTRUCTIONS

- Please enroll by calling MAC at (304) 292-3325 with enrollment & payment information or you can SIGN UP AND PAY ONLINE.
- Minimum enrollment must be met two days prior to the start date of a class, or the class  is subject to cancellation.
- Class size is limited to maximize learning. Enroll early to ensure your place!
- Fees will be refunded only if notice is received at least 24 hours prior to the beginning of the class. MAC does not prorate class fees.
- A complete refund will be made if MAC is required to cancel a class due to insufficient enrollment.
- It is MAC's policy that sick children and adults refrain from participating in classes or events.

MAC Publicity Policies MAC programs and events are often photographed and/or videotaped for publicity purposes. I understand and agree that footage taken of myself or my child at MAC or at a MAC sponsored activity/event may be used for publicity purposes.
Program Registration Release Agreement In consideration of acceptance of either my registration or my child’s registration, I hereby for myself, my child, my heirs, executors and administrators, waive and release any and all rights and claims for damages I or my child may have against the Monongalia Arts Center (MAC) Board of Directors and its representatives, for any and all injuries suffered by myself or my child during any activity sponsored by MAC.

Many of MAC's workshops and groups are free or have nominal costs under $5. These groups do not have pay options online since the fee is so small. Morgantown Poets, Song Writers Circle, Drum Circle, and MT Pockets' Playwrights groups typically ask for $2 donations, since the building is kept open and staffed passed regular business hours (after 7PM).
